Our Swing Bed program is a continuing care program for patients who no longer require acute care, but who are not yet ready to go home. This transitional program ensures that patients have access to the excellent care they need between hospitalization and full recovery.

Our goal is to help the patient transition safely and smoothly from hospital to home. We assist patients and families with details of their stay, provide ongoing contact and support, and assist with coordinating discharge.
